The Director of Care Coordination and Family Programs is
responsible for the oversight, organization, and administration of
a wide range of Fair Haven Community Health Care programs. The
Director promotes integration, continuity, and a data-driven
culture (using quality-improvement frameworks) to implement,
evaluate, and improve programs that address early childhood and the
social determinants of health.
Reporting to the Chief Medical Officer, the Director’s role is
approximately 60% administrative and 40% patient-facing. The
administrative roles involve overseeing the Care Coordination
Department, Family Home Visiting Partnership, WIC and other
critical components of clinical support services that form a social
safety network for a high-risk patient population. This individual
will work with the data team to make sure Care Coordination efforts
impact measurabed indicators of health, health care quality, and
satisfaction in care. The patient-facing roles involve outreach to
patients identified as having gaps in care or other vulnerabilities
and working with families to become re-engaged in care and to
address the social determinants of health. This may occur on site
or via home visits.
Duties and Responsibilities
Primary responsibilities include but are not limited to:
Oversee and lead the Care Coordination Department, insuring
standardization of workflows and integrated measurement of service
Provide administrative and programmatic guidance to the
Nurturing Families Network, WIC & Breastfeeding Services, Healthy
Start, and Reach Out and Read programs.
Manage gaps-in-care reports and implement Care Coordination
strategies to mitigate these gaps.
Work collaboratively with the senior leadership in furthering
FHCHC’s mission and strategic plan.
Collaborates with local and state stakeholders.
Represents FHCHC on community committees as assigned.
Manage productivity reports from the electronic health record
for Care Coordination encounters.
Implements and has responsibility for Care Coordination efforts
that will measurably achieve PCMH+ and UDS metric results.
Work with finance and grants management to manage required
Bachelor’s or master’s degree
Experience with gaps in care and data management
Experience working with teams and team management
Facility and experience with database management
Familiarity with quality improvement efforts
Bilingual in English and Spanish
Experience using EPIC electronic health record
