Job Description Job Description CASE MANAGERS NEEDED FOR
IMMEDIATE OPENINGS IN BROOKLYN, NY, working for an organization
that provides family shelter and supportive housing in NYC. This
position is Monday-Friday 9a-5pm role. Their mission is to
transform the lives of New York City homeless families by providing
holistic solutions of safe housing, critical services, and
ground-breaking programs they need to succeed on their own– so
these families can regain their independence and their children can
look forward to a brighter future. Their Vision is to break the
cycle of homelessness by being the premier leader and trusted
partner in innovative solutions for families in need. Duties: -
Provide case management services to individuals and families in
need - Conduct assessments to determine client needs and develop
individualized service plans - Coordinate and monitor the delivery
of services to clients - Advocate for clients and connect them with
appropriate resources and support systems - Collaborate with other
professionals, such as social workers, healthcare providers, and
educators, to ensure comprehensive care for clients - Maintain
accurate and up-to-date case records and documentation - Provide
crisis intervention and support as needed Required: - Bachelors
Degree preferred - Must have minimum of Associate Degree with 2
years of Case Management experience
