Job purpose The Ryan White Outreach, Prevention, and CQM Program Liaison, henceforth referred to as the RW Program Liaison Specialist, (RWLS), reporting to the Medical Director of HIV, HCV, and LGBTQ+ Care, will be responsible for assisting in the management of day-to-day operations and administration Ryan White HIV/AIDS and Infectious Disease programs and grants, working through FHCHC's multiple sites and promoting engagement of stakeholders across the multiple disciplines and departments. This individual will be responsible for programmatic development and quality improvement efforts that integrate initiatives related to population health, social determinants of health, and health equity, with a focus on HIV positive and LGBTQ individuals. She/he will also strive to uphold the mission statement of FHCHC as follows: To improve the health and well-being of the communities we serve through equitable, high quality, patient-centered care that is culturally responsive. The RWPLS will be instrumental in maintaining a strong presence in the community, developing and managing health education, health promotion, and wellness initiatives and strategies for medically complex and at-risk populations, again with a focus on HIV positive and LGBTQ+ individuals. The RWLS is able to work independently and collaboratively with FHCHC staff across multiple sites to plan, manage, and implement activities in the following areas: public health programming in prevention and health education; community"based outreach and community engagement; research and evaluation; quality improvement strategies; and resource and training development. Duties and responsibilities Program Management & Development: Works collaboratively with Medical director of HIV, HCV and LGBTQ+ Care to assess, design, implement, monitor, evaluate, and improve programs for HIV and LGBTQ+ care across the multiple sites and across the multiple disciplines and departments. Collect and maintain data as it pertains to Clinical Quality Measures outlined and required by Ryan White grant parameters Identifies the overall health/risk status of the HIV/Infectious Disease and LGBTQ populations being served at FHCHC and surrounding communities to plan, develop, implement, evaluate, and monitor health education and health promotion programs based on that assessment. Provides overall project management, including developing and managing timelines, leading project planning, and coordinating deliverables in collaboration with internal and external partners. Develops quality improvements strategies and monitors progress toward meeting program goals, while also improving performance and outcomes. Works closely with FHCHC data team to develop data driven strategy for quality improvement initiatives. Ensures program deliverables are met and there is no duplication of efforts. Helps identify programmatic trends, develops recommendations to guide quality improvement, and supports implementation of related workflow changes. Assists in maintaining data collection systems for quality improvement projects or other measurable program objectives. Develops, coaches, and mentors Ryan White, HCV, and LGBTQ+ staff, ensuring that personnel is properly trained and equipped to comply with grant requirements. Develops and maintains a strong CQM program and engages staff in the PDSA process. Participates in strategic planning and grant writing. Represents FHCHC at community meetings and events as assigned. Oversees the effort to integrate Ryan White services with other service lines at FHCHC by providing staff education and visibility to program initiatives. Develops and manages evidence-based community health programs, initiatives, and strategies to promote health and wellness among vulnerable and/or at-risk populations, with a focus on HIV-positive and LGBTQ individuals. Identifies and connects individuals within the community with substance and opioid addiction to Medication-Assisted Treatment services and counseling with a warm handoff to the addiction care coordinator. Identifies and connects individuals with mental health-related issues to coordinate warm handoff with Behavioral Health Team/Providers Coordinates manage and facilitate support groups and group educational sessions to ensure patient awareness of available services and linkage to these, working in partnership and collaboration with Medical Case Managers and Care Coordinators. Acts and leads as a community coach and health educator to facilitate access to health services and self-management of disease, promoting positive health behavior change. Provides and assures the mobilization and coordination of interagency and community partnerships for delivering innovative and effective public relations and health marketing to diverse audiences, with a focus on HIV positive and LGBTQ individuals, increasing the visibility and awareness of FHCHC's HIV/ID and LGBTQ+ healthcare programs to the community at large. Establishes linkages with community-based organizations as well as health and social resources available to community members to provide additional health education and health promotion programs and services. Staff Development: Coordinates ongoing continuing education and training for staff to facilitate their understanding, and competency in medical and behavioral health care issues related to HIV and LGBTQ+ Develops and maintains relationships with staff and management to efficiently and effectively implement and execute in-house competency training programs across multiple sites, tailored to the needs of each department and site. Develops streamlined processes and procedures for the MCM and HRSA reporting requirements of the Ryan White Grant and actively involved MCM in the CQM process. Develops CQM program in accordance with HRSA requirements and oversees ongoing execution of PDSA quality management projects. Identify the intersectionality of minority groups who differ from their cultural norms. These differences may include stigma due to HIV, gender identity, sexual orientation, immigration status or other barriers to care as outlined in the Mission Statement of FHCHC Qualifications Master's degree in public health, business, public administration, or related field strongly preferred. May be working toward a Master's degree and have 5 years' experience in health care. Demonstrated experience and expertise in quality improvement as well as program management and/or program development. Experience with non-profit agencies, preferably agencies involved with caring for HIV-positive and LGBTQ individuals. Must have strong knowledge of Microsoft office/EPIC experience preferred. Knowledge of Ryan White program preferred. Strong leadership, creativity, and organizational skills. Excellent oral and written communication skills. Flexibility and aptitude for problem-solving in a fast-paced work environment. Excellent computer skills including word processing, data entry, data analysis, and reporting. OSHA Status: Category III"Low-risk position Generally works in an office environment with no exposure to blood-borne pathogens.
