POSITION SUMMARY : Integrated Care Director implements and
oversees integrated medical services at BHcare while providing
leadership to staff and peers for primary care integration and care
coordination services for outpatient clients.
This is a full-time, 37.5 hours per week position. The
scheduled hours are Monday-Friday , 8:30 a.m. to 4:30 p.m.(some
flexibility to work evenings if necessary to meet the demands of
the program.) This position will be based at our Branford and
Accountable and responsible for the following results:
Oversees the coordination of care with internal and external
medical providers, health care agencies, specialty providers and
hospitals to ensure timely and appropriate care coordination and
primary care service delivery for clients.
Ensures BHcare meets the integrated primary care and care
coordination expectations of the all grants which require such.
Collaborates with clinic directors, program directors and
managers to ensure medical staff meets the expectations and goals
of all related grant and program requirements.
Oversees the development of wellness, health promotion &
prevention initiatives at both sites.
Supports staff in development of treatment/wellness plans and
healthcare goals for individuals with co-occurring chronic diseases
Provides oversight to the organization’s Infection Prevention &
Exposure Plan, including ongoing updates and maintenance of the
plan, education to staff, response to exposures, and oversight of
the Infection Prevention Nurse role.
May provide direct care and health consultation to clients.
Supervises nursing and medical assistants at both sites and
evaluates staff performance.
Collaborates with Fair Haven Community Health Center, other
primary care providers, and works to establish primary care
partners for care integration, particularly in the Valley area;
continually supports ongoing positive relationships with all

Works with CCBHC Program Director to evaluate and monitor
outcomes and plan performance improvements.
Ensures that all relevant data is entered into BHcare’s
electronic health record.
Ensures that all clients have health assessments, treatment
plans and maximize the usage of Targeted Case Management (TCM’s)
and Behavioral Health Home (BHH) Services to meet DMHAS’ billable

May be required to use own vehicle on company business or to

required for accreditation or licensing.
Perform other duties as assigned.
Regular and consistent attendance is an essential function of
Qualifications Current CT RN license. MSN required.
Excellent communication and writing skills.
Sensitivity to diverse cultures and language competencies.
Demonstrated experience in supervising primary care staff and

Demonstrated experience in field of behavioral health and
knowledge of psychiatric illness and substance abuse preferred.
Experience in primary care practice preferred.
Must be proficient with PCs and Microsoft Office Word & Outlook,
and ability to navigate through electronic health record.
Must have and maintain a valid CT driver’s license if relocated
to CT as a state resident more than 30 days ago. Must maintain an
acceptable driving record and be insurable at a reasonable rate
under the Organization’s auto insurance plan.
