Glengariff Healthcare Center, a premier facility located in Glen Cove, NY, is looking for a dedicated Licensed Practical Nurse (LPN) to join our team. Our center, which specializes in both short-term rehabilitation and long-term care, offers personalized treatment plans designed to meet the specific needs of each patient. In this role, you will work closely with our multidisciplinary team to ensure that all patients receive the highest quality of care and support in their recovery and daily living activities in our state-of-the-art facility. Administering medications and treatments to patients in accordance with healthcare plans Observing and reporting any changes in patients' conditions Assisting with patient mobility and physical therapies Ensuring that patient care plans are accurately followed and documented in medical records Collaborating with registered nurses, doctors, and other healthcare professionals to provide holistic care Educating patients and families on treatment plans and in-home care procedures Strong clinical skills in a healthcare setting Excellent communication and interpersonal skills Ability to follow detailed instructions and maintain patient confidentiality Proficient in using medical software and electronic health records Medical Insurance Dental Insurance Vision Insurance Valid LPN license in the state of New York CPR certification
