locum for ongoing coverage. Please list any anticipated
procedures/surgeries the provider will be required to perform and
anticipated volumes: C-sections D&C Description of Work
Assignment: In house OB coverage Breakdown of Clinic Days/Sessions
and/or OR Days:24 hour restricted call shifts Schedule of
Days/Times Needed to Work/ Shift Hours flexible On call? Restricted
call Patient Load 3-7 deliveries per day triage patients Board
