Experience: Two (2) years of experience in a HOME setting within
the last ten (10) years. License: Current RN Licensure in New York
State Certifications: BLS required before the start date Must-Have:
- All nurse assessors must meet the following requirements: - Two
(2) years of experience in a HOME setting within the last ten (10)
years. - Recent experience conducting comprehensive assessments of
patients. Description: Required qualifications include experience
working in the field for a certified home health agency, licensed
home care service agency, or managed long-term care. Candidates
should have experience conducting assessments that are similar to
established standards. Experience providing primary care to
homebound individuals is essential. Candidates should also have
experience managing care for persons receiving home and
community-based services including hospice and assisted living.
Preference will be given to those with additional experience
conducting assessments in the specified system, especially within
the last 365 days. Candidates should have experience caring for a
geriatric population and managing their chronic diseases,
particularly in a home setting. It is beneficial to have a
combination of home care experience as a paraprofessional or
licensed practical nurse along with RN experience. The following
examples do not meet the required qualifications but may count
toward preferred qualifications: Home care experience with only one
patient, such as private duty nursing. Case management limited to
utilization reviews for a health plan or a combination of hospital
and health plan case management. Assessments conducted exclusively
through telecommunication without home visits. Candidates will
conduct hybrid telehealth and in-field assessments. The
responsibilities include performing initial in-home pre-evaluations
of patients, reviewing consumer medical documentation, and entering
evaluation data into electronic forms. Documentation of any
concerns or conflicting information during the evaluation process
is required. It is important to emphasize continuity of care to
reduce fragmentation or gaps in treatment plans. Candidates will
discuss healthcare options with consumers regarding supports and
service vendor options. Initiating communication between relevant
parties for enrollment into managed care health plans is part of
the role. Enhancing communication and collaborative relationships
with team members is essential for improving care coordination.
Collecting quality review data and documentation to support outcome
measurements is required. Identifying opportunities for health
promotion and illness prevention falls within the responsibilities.
Candidates should maintain comprehensive knowledge of community
resources and services for the target population. Meeting
established performance criteria as outlined in performance
evaluations is expected. The role may include other duties as
