A Long Island nonprofit services agency is looking to add a new Licensed Practical Nurse (LPN) or licensed Registered Nurse (RN) to join their growing team at one of their residential facilities in Suffolk County.About the Opportunity:Shift: DaysSchedule: Monday to FridayHours: 7am to 3:30pmSetting: ResidentialPatient Population: 11 to 21 year old individuals with ID/DDResponsibilities:Develop a person-centered plan of care that prescribes strategies and interventionsSupport discharge planning process with members of relevant teamsProvide or coordinate the resident / family educationCollect comprehensive data pertinent to the resident's health or the situationPerform all other duties as assignedQualifications:6+ months of related Nursing experienceGraduate of an accredited School of NursingNYS Licensed Practical Nurse (LPN) and/or licensed Registered Nurse (RN)BLS certificationStrong understanding of the needs of vulnerable populations including youth and young adults with Autism, Developmental Disabilities, Communication Impairments, and Medical ComplexitySolid assessment, clinical and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedby Jobble
