Job Description Patient Led, Clinician Operated healthcare is seeing a like mindedPrimary Care Physicians to join us on their journey. This individual would beresponsible for managing patient care and treatment in collaboration with otherteam members. High Income Potential and Profit Sharing Outpatient primary care opportunity Full-time and Part-time opportunities Residents and experienced providers please apply $260,000.00 - $280,000.00 base pay Sign-on bonus Profit sharing Growth and leadership opportunities Responsibilities: Provide exceptional patient care Perform health assessments and screen for preventative healthmeasures Order, interpret and evaluate diagnostic tests to identify andassess patients' clinical problems and health care needs Formulate and document care plans Prescribe medication or other forms of treatment. Education: Board eligible or board certification in Internal Medicine,Family Medicine, or Geriatrics. Current CT license (or eligibility to apply) DEA license (or eligibility for registration) Demonstrated interest/passion for value-based care deliverymodels Strong oral and written communication skills
