Massapequa Center for Rehabilitation and Nursing - ADHC Program
RN
Massapequa Center's Adult Day Healthcare program is seeking a
part-time Program RN to include alternating weekends
Responsibilities include:
Follows accepted standards of nursing practice by using sound
scientific principles in -providing -direct care, performing
procedures and in using equipment. -
Provides assessment and nursing care to registrants on a routine
and crisis basis and monitors response to treatment and adjustment
to program. - -
Supervises LPN's and CNA's and performs performance
evaluations.
Completes initial UAS assessments and utilizes completed
assessments of all registrants within five visits, in a time period
not to exceed thirty days from registration, to develop a Person
Centered Service Plan (PCSP)
Completes scheduled re-assessments in accordance with NYSDOG ADHC
regulations as per departmental workflow. Utilizes completed
re-assessments to revise/update a Person Centered Service Plan
(PCSP). Responds to changes in member's condition/sentinel events
(i.e. level of care changes, changes in family support, changes in
housing, etc.) by revising the PCSP and communicating changes with
interdisciplinary care team and PCP
Develops, maintains and reviews person centered care plans to
insure compliance with NYSDOH ADHC regulations
Educates members on Advanced Directives/Health Care Proxy/Power of
Attorney (POA) and helps to facilitate the completion of such
documentation.
Coordinates all necessary registrant services with other
interdisciplinary team members and with outside medical and case
management providers at all times including during the
comprehensive care planning process.
Ensures nursing care and recommendations for follow up are
documented, dated, signed and properly maintained in the client's
record in a timely manner in accordance with NYSDOH ADHC
regulations. -
Address problems that may have an adverse effect on the client's
ability to respond to treatment -and ability to attain optimal
functioning including a review of Physicians instructions and
methods of optimization of ADL skills.
Reminds and if necessary assists registrants in the coordination of
medical/specialists appointments and follows-up to obtain report of
findings.
Ensures the provision and coordination of registrant teaching
related to maintaining registrant health - -
Incorporates Primary Care Provider's treatment orders into the
program's treatment of the registrant client -whenever
-appropriate. -
Sends copy of lab work ordered at ADHC to PCP/Clinic.
Conducts group with registrants to provide education, training, and
support related to the health needs of the registrants. -
Administers STAT medication and inoculation and renders treatment
as prescribed by Physician or Nurse Practitioner.
Participates in Quality Assurance Performance Improvement programs
to assess, evaluate, revise and monitor the effectiveness of
various health related protocols. - -
Reviews client's medical history as part of the initial evaluation
and at the 180 day reassessment period. - -
Reviews and notes changes in the client's prescription on the
medical sheet as well as assesses client compliance. Incorporates
compliance and education issues into care plan if necessary. -
Acts as liaison to client's family, significant others, etc---. in
the -communication of health care information. -
Requirements:
Must have a valid RN license by NYS
Must have at least 1 year of experience working in a ADHC
program
Must be able to complete assessments in UAS
