Job Description
Collaborates with clinical staff to manage the referral of patients
within the Health System. Provides referral resources to physicians
and other staff throughout the Health System. Liaises with clinical
staff to coordinate patient care and ensure patients a seamless
process. Assists patients with service related issues to enhance
overall patient experience.

Job Responsibility
1.Communicates with patients and physicians to identify appropriate
referral based on patient need.
2.Assesses and understands patient needs and advocates on behalf in
the outpatient, ambulatory setting, and hospital environment.
3.Identifies cultural and social-specific issues and addresses
issues appropriately.
4.Processes, tracks, and documents patient care throughout the
patient experience.
5.Identifies and secures appropriate referrals based on patient
need in collaboration with Northwell NetworkCare staff.
6.Participates in operational processes related to referrals
including identifying programs to enhance processes and client
satisfaction.
7.Ascertains demographics, insurance information, and pertinent
details from patients and physicians to promote efficiency and
enhance the referral process.
8.Identifies, contacts, informs and follows-up with patients
throughout care experience to create a seamless patient
experience
9.Assists patients in solving issues related to health care
systems, financial areas, social barriers (ie: interpreter request,
transportation services, documentation for next steps, etc)
10.Distributes marketing material and communicates the mission of
Northwell NetworkCare to professionals and patients.
11.When applicable visits with hospitalized patients as a
representative of Northwell NetworkCare.
12.Performs related duties, as required.
Job Qualification
1.Associates Degree required; Bachelor's Degree, preferred.
2.Minimum of three (3) years clinically-related experience,
required.
3.Strong customer relationship skills, interpersonal and problem
solving skills, required.
4.Computer proficient with Microsoft Office, Excel, Power Point
5. Working knowledge of AllScripts and/or Sunrise strongly
preferred
6. Knowledge of medical terminology, and prior experience in a
clinical environment strongly preferred
