You will support the clinical team in implementing member care plans and coordination of member care needs and will also assist with location of members in the community. - Primary Responsibilities:

  • Partner with clinical care management team to manage complex members in the community
  • Conduct assessments, face to face or telephonically, to assess member medical, behavioral, functional, cognitive, and psychosocial needs, i.e., health risk assessment -
  • Research, locate and make referrals to community resources to address member specific care or socioeconomic needs. -Follow up to confirm services are received
  • Proactively engage the member to manage their care, provide education and coaching to support self-management of needs and health promotion as outlined in care plan

    • High School Diploma / GED (or higher)
    • 2+ years of experience working within the Community Health setting in a field-based healthcare role
    • 2+ years of experience working within the local Community, with demonstrated knowledge of culture and values within the Community and familiarity with the resources available within the Community
    • 1+ years of experience with MS Office, including Word, Excel, and Outlook
    • Highspeed Internet access in your area -
    • Designated work area
    • Reliable transportation and the ability to travel within assigned territory to meet with members and providers
    • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance.       Preferred Qualifications:
      • Bachelor's Degree (or higher) in Social Work or related subject
      • LPN, CNA, CMA
      • CHW Accreditation
      • CCM certification
      • Experience working in team-based care
      • Background in Managed Care
        Physical Requirements:
        • Ability to transition from office to field locations multiple times per day
        • Ability to navigate multiple locations/terrains to visit employees, members and/or providers
        • Ability to transport equipment to and from field locations needed for visits (ex. Laptop)
        • Ability to remain stationary for long periods of time to complete computer or tablet work duties
