Overview:

Promotes wellness initiatives by encouraging members of the community to participate in screenings and assessments.EEO/AA/Disability/Veteran

Responsibilities:
1. -Provides client company wellness services to the City of New Haven on behalf of Yale-New Haven Hospital.2. -Recruits patients for participation in various employee wellness programs.3. -Encourages client company employees to utilize the YNHH clinics availble to them and encourages wellness by promoting the use of free screenings and other wellness related programs offered by YNHH4. -Responsible for ensuring appropriate follow-up care and compliance with medical recommendations for the patient's wellness.5. -Responsible for promoting the YNHH wellness services at off-site locations for occupational health client companies.6. -Documents all client and community interactions. Responsible for creating reports summarizing screening and counseling activities7. -Provides screening services for client company employees.

Qualifications:
EDUCATION Bachelor's in nursing required; Master's in nursing or related healthcare related field strongly preferred. EXPERIENCE Two years relevant experience in providing community health education, health promotion activities, outreach, and/or patient navigation. LICENSURE Valid CT State Driver's License SPECIAL SKILLS Excellent oral and written communication skills. Excellent interpersonal and organizational skills, statistical and computer knowledge. Knowledge of principles of teaching the adult learner.
Ability to effectively connect with individuals and encourage the utilization of free medical services provided by YNHH to improve the health of the community. Familiarity with the Microsoft Office Suite. PHYSICAL DEMAND Travel is required.
