Mary Wade is seeking experienced LPNs for the following shifts:
Full-Time: 32 hrs/wk: 3 pm to 11 pm12hrs/wk: 7am to 7 pm: Every
Other Weekend8 hrs/wk: 7 am to 3 pm: Every Other Weekend8 hrs/wk: 3
pm to 11p: Every Other Weekend8hrs/wk: 7 am to 3 pm: Every
Sunday8hrs/wk: 3 pm-11 pm: Every Other Sunday8 hrs/wk: Every Other
Friday: 3 pm to 11 pmHourly Rates: $29.00 to $35.00At Mary Wade you

have 94 beds, 3 shifts, every other weekend rotations and shift

- Oversee overall operation and optimal quality of care for the
residents on the assigned unit.
- Follows all health, sanitary and infection control policies and
maintains established standards of practice set forth by the
facility's administration and Nursing Policies and Procedures.
- Holds daily shift report with nursing assistants assigned to
the unit prior to the start of the shift.
- Reviews written daily assignments for each resident on assigned
unit.
- Assures assignments are completed before the end of the
shift.
- Collaborates with Unit Manager to develop and maintain nursing
care plans and update these plans on a monthly basis or more often
if the resident's condition changes.
- Assumes responsibility in supporting the Unit Manager, DCS,
ADCS in promoting and maintaining positive employee relations and
effective communications among facility staff.
- Displays a polite and courteous manner to all residents and
visitors.
- Contacts the resident's physician and family to notify them of
a change in the resident's condition or accident/incident; also
notifies the supervisor covering.
- Participates in the annual evaluations of nursing assistants
and documents accurately their strengths, weaknesses and the steps
taken to improve their performance.
- Assumes independent responsibility for documenting disciplinary
concerns for nursing assistants on designated shifts.Qualifications
- Current Connecticut LPN License
- Minimum of 1 year experience in a Long Term Care Skilled
Nursing Facility
- Flexibility with schedule
- Ability to work every other weekendsCovid Vaccine and Booster
