Job Description:

The Case Manager is responsible for providing home-based case management services for families experiencing homelessness who are rehoused in apartments throughout Long Island. The primary goal is to assist families in addressing the barriers to maintaining permanent housing using a strengths-based, Housing First approach. Functions performed include housing location, family goal planning, connections to community resources, home visits, rental payment monitoring, landlord engagement and problem solving with other case managers.Essential Functions:- Make initial referrals for current shelter clients to RRH- Coordinate with Housing Specialist to locate housing options for families- Develop and coordinate financial goals with Financial Advocate- Complete home visits, meet with shelter clients and/or Zoom calls as needed to maintain support of clients- Work with families to resolve landlord issues, as needed- Provide referrals and resources to clients, as needed- Maintain current records in Apricot, documenting interactions with families- Capture daily and monthly statistics and other required reports as directed- Participate in events and/or meetings, as needed- Other duties are required $1,000 SIGN ON BONUS Job Type: Full-timeBenefits: 401(k) Dental insurance Health insurance Paid time off Vision insuranceSchedule: 8 hour shift Day shift Monday to FridaySupplemental pay types: Signing bonus
