Serves as a direct link between YNHHS Executive leadership and the managers and staff on the transplant patient care teams/departments. Under the direction of the Vice President of Heart and Vascular Center (HVC) and Yale New Haven Transplantation Center (YNHTC) Services, the Transplant Director is responsible for the direct leadership for managing transplant practices and programs within YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice; assuring comparable standards of care throughout the continuum of care across the Yale New Haven Health System (YNHHS). Provides strategic and operational leadership for the clinical and administrative staff and operations of the YNHTC; Living Donor Centers. Accountable for all facets of transplant operations including managing patient demand and throughput, patient satisfaction, regulatory preparedness, quality improvement, physician satisfaction, employee satisfaction, business development, and fiscal stability across YNHHS. Responsible for promoting an operation and culture that focuses on patient and family-centered care and on enhancing a world class model of seamless quality and cost effective patient care 24/7 both operationally and clinically. In addition, the Transplant Director will assist in the development of a self -directed work team for providing safe, effective, family focused care to patients who require transplant services. This includes quality assessment, clinical development, and education of the members of the transplant team. The Transplant Director will also serve as a liaison with the YNHTC Service Line and assure consistency in UNOS and the regulatory requirements as needed. In addition, the Transplant Director has responsibilities towards both public and professional education about the transplant process. Specific activities and responsibilities flex with the strategic planning of the YNHTC Program, its operations and nursing practice across the YNHHS. The Transplant Director has the authority to formulate and execute program vision, goals, and plans consistent with the organizations strategies and policy. Determine whether needed resources have been appropriately committed and facilitate the deployment of additional resources where indicated. Decisions regarding change to practice are developed and implemented in collaboration with the Vice President of HVC and YNHTC Services as necessary.

• 1.--Directs clinical practices and programs throughout the YNHHS, assuring comparable standards of care throughout the continuum of care. In collaboration with the Vice President of HVC and YNHTC Service Lines and the Section Chief's, the Transplant Director defines models of nursing practice, determines nursing resources and education requirements and assures the highest quality standards for clinical practices across the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice.
• 2.--Coordinates initiatives to measure and improve the quality of clinical programs to prepare for surveys/inspections by various regulatory agencies; ensure that desired patient outcomes are achieved through safe and efficient utilization of resources and are compliant with UNOS and Medicare requirements/regulations.
• 3.--Oversees the ongoing assessment of, compliance with TJC, State, Local and Federal regulations for all clinical services and communicates all relevant regulatory standards to staff, ensuring that standards are met or exceeded and facilitates the evaluation of coordination of process improvements to lead clinical staff performance that consistently exceeds accreditation requirements.
• 4.--Collaboratively facilitates and defines models of nursing practice; facilitates and monitors the development and implementation of core processes and systems of care which achieve strategically relevant volume and quality outcomes in the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice across the YNHHS.
• 5.--Identifies and analyzes trends in satisfaction, quality outcomes, and practice excellence using data systematically to guide the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice across the YNHHS.
• 6.--Manage the development and execution of clinical, educational, administrative and quality projects while providing support to the administrative team in the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice.
• 7.--Maintain a matrix management relationship with the leadership and ancillary staff who are dedicated to organ transplantation [i.e., pharmacy, social work, nutrition] and will assume responsibility for facilitating positive interactions with all departments and personnel involved in the transplant process.
• 8.--Organize and manage the multi-disciplinary team responsible for coordination of all indirect and direct transplant patient care management in the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice.
• 9.--Provides leadership and mentoring to the transplant team by demonstrating excellence in inpatient and outpatient transplant coordination and nursing practice; oversees and facilitates the development of leadership talent and new managers as necessary.
• 10.--Participates in developing a highly skilled multidisciplinary team who are utilized effectively; coordinates regular and on-call staff scheduling.
• 11.--Provides vision and leadership to staff in a collaborative environment that offers job satisfaction, recognition, and stimulates innovative thinking to accomplish goals and objectives. Oversees and facilitates that budgetary needs are addressed to allow continuing education as per CMS regulations.
• 12.--Takes responsibility for identifying areas for self-development and actively seeks opportunities and resources to meet developmental objectives.
• 13.--Partners with program directors and human resources, oversees and ensures that systems /processes are in place for the recruitment and retention of adequate and appropriate human resources to meet patient, regulatory, fiscal, and developmental requirements for the YNHTC Program across the YNHHS.
• 14.--Responsible for the growth and development of the clinical transplant coordinators, nurses and support staff within the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice; assess and identify learning needs of the clinical coordinators and support staff and provides advice and direction for future development; and implements orientation programs for new staff members across the YNHHS .
• 15.--Ensures all staff members understand their roles in accountability for achieving high levels of performance consistent with evidence-based practice; accomplishing program, divisional, and strategic objectives; and held accountable for adhering to patient relation and service excellence standards for YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice.
• 16.--Promotes the organization to all customers by interpreting and communicating Yale New Haven Health
• 17.--System mission and values, acting as a loyal, supportive, and informed spokesperson for the program, division, and organization.
• 18.--Participates in the YNHHS committee meetings; YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice committee meetings; and relevant community programs, healthcare agencies, charities, and professional organizations as a representative of the institution.
• 19.--Identifies and communicates opportunities for collaboration with other departments and the medical staff.
• 20.--Participates in implementing strategic efforts by influencing physician leadership and championing projects within the program, allocating resources, and utilizing project management skills.
• 21.--Partners with the Vice President of HVC and YNHTC Service Lines to manage resources and allocate staff throughout the YNHTC; Living Donor Centers, inpatient/ambulatory operations and nursing practice across the YNHHS.
• 22.--Participates in financial forecasting and planning along with other members of the management team.
• 23.--Evaluates the program operation for potential implementation of cost effective changes. Ensures that required resources are available (e.g. staff, technology) to meet program priorities and reflect staff and physician input.
• 24.--Participates in the discussion of financial responsibility for complicated patient cases without current insurance coverage and ensures that the financial coordinator secures insurance coverage and patient accountability prior to transplantation

Patient Services Division Leadership Qualifications EDUCATIONMaster's Degree in a healthcare related field required.EXPERIENCEMinimum of 8 years of progressive experience in healthcare leadership along with significant operational responsibility. Minimum of 5 years demonstrated successful leadership/management experience in acute care setting and Inpatient/Ambulatory/Outpatient for transplant practice management. Experience with management of comprehensive scope of responsibilities and FTEs (greater than 200 or more). Academic Medical Center and Transplant experience preferred. Thorough knowledge of Magnet status requirements as it relates to the certification processSPECIAL SKILLS1. Microsoft Office and computer skills required. 2. Advanced and diverse leadership knowledge and skills are required. 3. Excellent analytical, interpersonal, problem solving, time management, written, oral communication skills are required. 4. Ability to train and supervise employees with clear and accurate direction and guidance that represents YNHH positively and respects diversity is required.
