Job Description Job Description $ 42.4792 to $ 52.2243 per hour,
depending on actual experience The Social Services Department in
Manhattan Division of The New Jewish Home is seeking for a full
time temporary Social Worker to join their team! The duration of
work is between 4 to 5 months and maybe extended if needed. Job
Responsibilities: Completing MDS and biopsychosocial assessments.
Facilitate care plan meetings & discharge planning. Implement
Advanced Directives and End of Life Planning. Completing all
required documentation in the medical record ( EMR) Coordination of
care and services including ongoing monitoring of an appropriate
and effective person-centered care plan; Strong Clinical,
Communication and Organizational Skills Qualifications:
Demonstrates judgment of safety and confidentiality issues Strong
verbal and written communication skills Strong customer service
focus required Knowledge of the community resources Social work
Long term care experience preferred. Master Degree in Social Work,
licensed (LMSW)
